Job Title: Full-Time or Part-Time Physical Therapy Assistant – In-Home Mobility Expert

Company: Home Health Care, Inc.

Schedule: Part-Time with weekend rotation

Compensation: $42 - $46 per visit + mileage reimbursement

What You’ll Do:

  • Implement personalized treatment plans developed by a licensed Physical Therapist, helping patients regain strength, range of motion, and independence
  • Deliver hands-on care including therapeutic exercises, mobility training, gait education, and adaptive strategies to support recovery
  • Document all interventions and patient responses in EMR to track clinical progress
  • Educate patients and caregivers on home exercise routines and fall prevention strategies, empowering them with confidence
  • Collaborate regularly with the supervising PT and interdisciplinary care team to ensure holistic patient care

Qualifications:

  • Licensed Physical Therapy Assistant in the state of Minnesota
  • Valid driver’s license , reliable vehicle, and current insurance
  • Strong communication skills , with the ability to educate and motivate patients
  • Home care experience is a plus—but we will train the right individual passionate about making a difference
