Job Title: PA-C – Home-Like Primary Care for Senior Communities

Compensation: $105,000 to $120,000

Benefits: Full Benefits Package

Position Summary:
We’re looking for a Physician Assistant (PA-C) who believes in person-centered, place-based care . Instead of a rushed clinic setting, you’ll have time to connect with patients in their own environment—where your care can truly change outcomes.

Responsibilities:

• Provide routine and follow-up primary care to older adults
  
• Collaborate with care teams and support staff on treatment planning
  
• Monitor chronic conditions and acute medical concerns
  
• Use EMR to document care clearly and consistently
  

Qualifications:

• MN PA license in good standing
  
• Experience in geriatrics, care transitions, or chronic disease management
  
• Compassionate and collaborative
  
• Valid driver’s license and auto insurance

• Army: 65D – Physician Assistant
  
• Navy: PA – Physician Assistant
  
• Air Force: 42G1 – Physician Assistant
  
• Marines: PA – Physician Assistant
  
• Coast Guard: PA – Physician Assistant
